The following are the forms needed for students to arrange an externship at a practice or to request an NDSU Veterinary Technician Extern to come to your practice.

Student Guidelines for conduct during the Externship - Externship Guidelines

Letter to be sent to the Practice explaining the externship requirements - Externship Practice Letter and for practices that have had NDSU Vet Techs in the past you will need this information sheet - Established Externship Practice Information Sheet

If you are applying to a practice that has NEVER had an NDSU Vet Tech Extern you will need this information sheet - New Externship Site Information Sheet - This sheet can also be used by Practices that would like to be included in our Practice Book for Externs.

This form needs to be taken with to the Externship if going to Established Externship Sites - Externship Equipment & Procedures Skills List this list needs to be filled out and sent back with the Practice Information Letter. The equipment list and procedures are included in the New Externship Site Information Sheet so there is NO need to print this out if applying for inclusion in the NDSU Externship Practice Book.

Evaluation Forms - there are two, one for the practice to evaluate the Extern and one for the Extern to evaluate the Practice. Both need to be returned after the externship is completed. Evaluation of the Extern Student and Evaluation of the Practice
